Your role and responsibilities 

  • hold a caseload, which will include complex and challenging work, supporting the residents of Suffolk 
  • undertake home visits when required and provide detailed information and advice in supporting people to remain independent and safe in their own homes 
  • liaise with other professionals as required 
  • to work within budgetary restraints to achieve value for money for customers, carers and the service 
  • to develop skills under the guidance of Social Workers to recognise and work effectively with complex social situations which may include Adult Safeguarding.
  • to develop skills under the guidance of Occupational Therapists to assess functional ability with an emphasis on reablement and develop clinical reasoning skills to advise on Daily Living Equipment and adaptations. 
  • champion the use of Assistive Technology including the Digital Care solutions
  • support the triage of new referrals.

 

You will need 

  • experience of working with people within social care/health settings - either statutory, voluntary or charity
  • experience of person-centred work 
  • a keen focus on preventative social care and a creative mindset in achieving good outcomes 
  • an understanding of risk assessments 
  • the ability to work independently as well as part of a team 
  • excellent communication skills and an awareness and understanding of people’s emotions and reactions.

The team 

The Independence and Wellbeing Service (IWS) is the front door Social Work team, working alongside Customer First to offer robust advice, information, equipment, digital care aids at the earliest point of an individuals journey into Adult Social Care. There are three IWS teams in Adult Social Care, located in Waveney, West Suffolk and Ipswich. 

IWS work involves, but is not limited to;

  • arranging reablement services
  • undertaking short term assessments
  • mental capacity assessments
  • home visits where appropriate 
  • commissioning services
  • reviewing support.
